Today we will be discussing about high blood pressure, methods to reduce high blood pressure, dyslipidemia which is abnormal fat and cholesterol levels in your blood and measures to correct high levels

Blood Pressure:
A blood pressure reading measures the force of blood as it presses against the inside walls of the blood vessels. Blood pressure is written as 2 numbers:
o Systolic blood pressure (top number) is the force when the heart pumps.
o Diastolic blood pressure (bottom number) is the force between heart pumps.
Hypertension describes a condition when blood pressure values are persistently above normal range. One out of every four persons in India is diagnosed with hypertension. High blood pressure is more common in people with diabetes. More than half of people with Type 2 diabetes eventually develop high blood pressure. Hypertension may not cause symptoms, but over time it damages the heart, other organs, and blood vessels. High blood pressure is dangerous because it makes your heart work too hard and contributes to hardening of the arteries (atherosclerosis). It increases your risk of heart disease and stroke, which are leading causes of death in India. High blood pressure also can result in other conditions, such as congestive heart failure, kidney disease, and blindness. All these ill-effects of hypertension are more pronounced in patients with diabetes.
For patients with diabetes, the blood pressure goal is less than 130/80 mm Hg. Apart from medications that your treating physician may prescribe for keeping blood pressure under control, it also crucial to cut your salt intake. It is recommended to limit your salt intake to less than 6,000 mg (6 g) per day. There is no necessity to eat a completely salt free diet. Cook food with minimum salt and avoid adding additional salt to food at the dining table, curds, buttermilk, salad etc. Foods with high salt content have to be restricted to keep sodium intake at the optimum levels like pickles, papad, namkeen, processed foods, Chinese salt (ajinomoto or monosodium glutamate), dried sea food, bakery foods, sauces, ketchup and cooking soda. Instead, use various spices and herbs to enhance the taste of your food like lime juice, tamarind, tomato, vinegar, ginger, garlic and mint. Other important measures include having good quantity of fruits, vegetables and low-fat dairy products in your diet, having regular exercise and maintaining body weight that is in the healthy range.
Cholesterol & Fats:
Cholesterol is a waxy, fat-like substance that is in all the body's cells, including the blood. Your body needs cholesterol to make some hormones, vitamins, and to help you digest. Your body makes all of the cholesterol it needs. Cholesterol is also found in some foods you eat.
However, as with many things in life, too much of a good thing can become bad for you over time.
In the blood, cholesterol is carried in small packages called lipoproteins (lip-o-PRO-teens). Two kinds of lipoproteins carry cholesterol through your body. It's important to have healthy levels of both LOW-density and HIGH-density lipoproteins.
LDL (bad) cholesterol can damage arteries. Patients with diabetes may have higher the usual levels of LDL cholesterol. It may be especially dangerous to artery walls and often cannot be fully corrected through blood sugar control. When too much LDL cholesterol is in your blood, it may be deposited in the inner walls of your arteries. Together with other substances, it can form plaque and cause your risk of heart disease to increase. So keep your LDL cholesterol level below 100 mg/dL. In some cases, if you have other cardiovascular risk factors, your healthcare provider may want your level to be below 70 mg/dL
HDL (good) cholesterol works to clear cholesterol from the blood vessels, helping to keep arteries healthy. In patients with diabetes, HDL cholesterol levels are often lower than normal, increasing the risk of artery damage. Your HDL cholesterol levels should not be below 40 mg/dL (the higher the better). An HDL cholesterol of 60 mg/dL and above is considered protective against heart disease.
Triglycerides are another type of fat in the blood. Having type 2 diabetes can lead to elevated triglyceride levels. Elevated levels of triglycerides in turn can further increase the risk of heart disease. You can keep triglycerides under control by avoiding high fat foods, refined sugars and simple carbohydrates and alcohol
Targets in patients with diabetes are:
o HDL cholesterol > 40 mg/dL (men) or > 50 mg/dL (women)
o LDL cholesterol less than 100 mg/dL (

आज हम उच्च रक्तचाप, उच्च रक्तचाप को घटाने के तरीकों, डिसलिपिडेमिया जो आपके खून में असामान्य वसा और कोलेस्ट्रॉल का स्तर है, तथा उच्च स्तर को ठीक करने के उपायों के बारे में चर्चा करेंगे.

रक्तचाप:
रक्त द्वारा रक्त-वाहिकाओं की अंदरूनी दीवारों पर डाले जाने वाले दबाव को रक्तचाप का माप कहा जाता है. रक्तचाप को 2 संख्याओं के रूप में लिखा जाता है:
सिस्टोलिक रक्तचाप (ऊपर की संख्या) वह शक्ति है जब दिल पंप करता है.
डाईस्टोलिक रक्तचाप (नीचे की संख्या) दिल द्वारा दो बार पम्प किये जाने के बीच की शक्ति है.
उच्च रक्तचाप उस स्थिति को दर्शाता है जब रक्तचाप की वेल्यू सामान्य सीमा से लगातार ऊपर हो. भारत में हर चार व्यक्तियों में से एक उच्च रक्तचाप से पीड़ित पाया जाता है. मधुमेह से ग्रस्त लोगों में उच्च रक्तचाप अधिक पाया जाता है. आधे से अधिक टाइप २ मधुमेह से ग्रस्त लोग अंततः उच्च रक्तचाप के शिकार हो जाते हैं. हो सकता है उच्च रक्तचाप लक्षण प्रकट न करे, लेकिन समय के साथ यह दिल, अन्य अंगों तथा रक्त वाहिकाओं को हानी अवश्य पहुंचाता है. उच्च रक्तचाप खतरनाक है क्योंकि यह आपके दिल को काफी अधिक मेहनत करने पर मजबूर करता है, और धमनियों के कड़े हो जाने (एथिरोस्केलेरोसिस) में भी योगदान करता है. यह हृदय रोग और स्ट्रोक की संभावनाओं को बढ़ा देता है, जो कि भारत में मृत्यु के प्रमुख कारणों में से हैं. उच्च रक्तचाप कंजेसटिव हार्ट फेल्यर, गुर्दे की बीमारी, और अंधापन, जैसी अन्य स्थितियों का कारण भी हो सकता है. उच्च रक्तचाप के ये सभी दुष्प्रभाव मधुमेह के रोगियों में अधिक स्पष्टता के साथ सामने आते हैं.
मधुमेह के रोगियों के लिए, रक्तचाप लक्ष्य १३०/८० एमएम एचजी से नीचे होना चाहिए. रक्तचाप पर नियंत्रण हेतु आपके चिकित्सक द्वारा बताई गयी दवाओं के अलावा, अपनी नमक सेवन की मात्रा में कमी करना भी अत्यंत महत्त्वपूर्ण होता है. यह सलाह दी जाती है कि आपके नमक सेवन की मात्रा को ६,००० मिलीग्राम (६ ग्राम) प्रति दिन से कम होना चाहिए. पूर्णतया नमक मुक्त भोजन खाने की भी कोई आवश्यकता नहीं है. भोजन को कम से कम नमक के साथ पकाएं और खाने की मेज पर दही, छाछ, सलाद आदि में अतिरिक्त नमक डालने से बचें. नमक की उच्च मात्रा वाले खाद्य पदार्थों को कम करना चाहिए ताकि सोडियम की मात्रा को उचित स्तर पर बनाये रखा जा सके. ऐसे खाद्य पदार्थों में शामिल हैं- अचार, पापड़, नमकीन, प्रसंस्कृत खाद्य पदार्थ, चीनी नमक (अजीनोमोटो या मोनोसोडियम ग्लूटामेट), सूखा समुद्री भोजन, बेकरी खाद्य पदार्थ, सॉस, चटनी और कूकिंग सोडा. इसके बजाय, अपने भोजन का स्वाद बढ़ाने के लिए आप नींबू का रस, इमली, टमाटर, सिरका, अदरक, लहसुन और मिंट जैसे विभिन्न मसालों और जड़ी बूटियों का प्रयोग करें. अन्य महत्वपूर्ण उपायों में शामिल हो सकते हैं, आपके आहार में फलों, सब्जियों और कम वसा वाले डेयरी उत्पादों का अच्छी मात्रा में शामिल होना, नियमित व्यायाम करना और शरीर के वजन को एक स्वस्थ सीमा के अंदर बनाये रखना.

कोलेस्ट्रॉल एक मोम तथा वसा जैसा पदार्थ है जो रक्त समेत शरीर की सभी कोशिकाओं (सेल) में मौजूद रहता है. आपके शरीर में कोलेस्ट्रॉल की जरूरत कुछ हार्मोन तथा विटामिन का निर्माण करने तथा पचाने में मदद के लिए पड़ती है. आपका शरीर आवश्यक कोलेस्ट्रॉल की पूर्ण मात्रा का निर्माण स्वयं ही करता है. कोलेस्ट्रॉल आपके द्वारा खाए जाने वाले कुछ खाद्य पदार्थों में भी पाया जाता है.
हालांकि, जैसा कि जीवन में कई चीजों के साथ होता है, किसी अच्छी चीज की अति भी समय के साथ आपके लिए बुरी बन सकती है.
रक्त में कोलेस्ट्रॉल का वहन लिपोप्रोटीन नामक छोटे छोटे पैकेटों द्वारा किया जाता है. आपके शरीर में कोलेस्ट्रॉल का वहन दो प्रकार के लिपोप्रोटीन द्वारा किया जाता है. यह महत्त्वपूर्ण है कि दोनों निम्न-घनत्व और उच्च-घनत्व लिपोप्रोटीन का स्वस्थ स्तर बना रहे.
एलडीएल (खराब) कोलेस्ट्रॉल धमनियों को नुकसान पहुंचा सकता है. मधुमेह के मरीजों में एलडीएल कोलेस्ट्रॉल का स्तर सामान्य से अधिक हो सकता है. यह विशेष रूप से धमनियों की दीवारों के लिए खतरनाक हो सकता है, और प्रायः ब्लड शुगर नियंत्रण के माध्यम से पूर्णतया सुधारा नहीं जा सकता है. जब आपके रक्त में एलडीएल कोलेस्ट्रॉल अत्यधिक मात्रा में हो, यह आपकी धमनियों की आंतरिक दीवारों में जमा हो सकता है. अन्य तत्वों के साथ मिलकर, यह प्लेक का निर्माण करके हृदय रोग के खतरे को बढ़ा सकता है. अतः, अपने एलडीएल कोलेस्ट्रॉल के स्तर को १०० मिग्रा / डीएल से नीचे ही रखें. कुछ मामलों में, यदि ह्रदय संबंधित कुछ अन्य जोखिम कारक भी मौजूद हैं तो आपका चिकित्सक सलाह दे सकता है कि आपका स्तर ७० मिग्रा/डीएल से नीचे ही बनाये रखा जाये.
एचडीएल (अच्छा) कोलेस्ट्रॉल रक्त वाहिकाओं से कोलेस्ट्रॉल को साफ करके धमनियों को स्वस्थ बनाये रखने में मदद करता है. मधुमेह के रोगियों में, एचडीएल कोलेस्ट्रॉल का स्तर अक्सर सामान्य से कम होता है, जिससे धमनी को नुकसान का खतरा बढ़ जाता है. आपके एचडीएल कोलेस्ट्रॉल के स्तर को ४० मिग्रा/डीएल से कम नहीं होना चाहिए (जितना अधिक हो उतना ही अच्छा). ६० मिग्रा/डीएल से अधिक एचडीएल कोलेस्ट्रॉल को ह्रदय रोगों के खिलाफ अच्छा समझा जाता है.
ट्राइग्लिसराइड्स रक्त में वसा के एक अन्य प्रकार हैं. प्रकार २ मधुमेह उच्च ट्राइग्लीसराइड स्तर का कारण हो सकता है. ट्राइग्लिसराइड्स के बढ़े हुए स्तर हृदय रोग के खतरे को और बढ़ा सकते हैं. उच्च वसा वाले खाद्य पदार्थ, परिष्कृत शर्करा और सरल कार्बोहाइड्रेट तथा शराब से परहेज द्वारा आप ट्राइग्लिसराइड्स पर नियंत्रण रख सकते हैं.
मधुमेह के रोगियों में लक्ष्य इस प्रकार होने चाहिए:
एचडीएल कोलेस्ट्रॉल > ४० मिग्रा/डीएल (पुरुष) या > 50 मिग्रा/डीएल (महिला)
