Swedish to English Bus/Financial Management
Avvikelse is of course ordinarily translated as “discrepancy,” or some kind of divergence. I am looking however for a word that represents when something goes wrong, like an accident, or something not working out right, that still would be within the meaning of the word avvikelse.

This is another one that gets used (deviating from the norm). or anomaly, depending on the context.

My thesaurus also comes up with kinkiness... but that may not fit your context!!!

A suggestion. Or perhaps 'departure', depending on the context. :)

incident

You could well translate e.g. avvikelserapport by incident report.
Another alternative to the boring (though correct) deviation is nonconformity, which is the term used in quality assurance.

Just another option, depending on the overall subject/style, etc. Deviation is also correct.
