does it mean these services have to be paid as an extra

I would leave the first phrase "against cost" as it is.

In the last sentence I would use "against costs incurred" ; and yes, this does mean that additional costs have to be paid (extra).

yes, money is charged for additional building components

is what is sounds like to me

additional cost based on time and materials

These are add-ons whose specific costs not provided for in the contract.
