HPI (History of the present illness)
https://goo.gl/lMduqy

"Not to be confused with Past medical history.
Following the chief complaint in medical history taking, a history of the present illness (abbreviated HPI)[1] (termed history of presenting complaint (HPC) in the UK) refers to a detailed interview prompted by the chief complaint or presenting symptom (for example, pain)."

: I've seen several hundred online instances of "interrogatorio intencionado" ... so possibly it does have a specific meaning (which escapes me) in Spanish usage. I would just use "interview" or "questioning" as you suggested.

Joseph, I have also seen those many instances of "interrogatorio intencionado", but that doesn't mean it is correct. All interrogatories have an intention (to gather information), so I believe it is redundant to say "intencionado".

: This makes much more sense to me, and you've given examples this time. I still wonder whether "intencionado" has some special meaning in this context. Esta respuesta me parece mucho mejor.

: Sounds like an interesting possibility. Now ... how do we know what a "directed history" is in English? It would help if you could find a definition, and then show that it matches the meaning of "intencionado."

• Interrogatorio con intención diagnóstica. n o o d i n r to o t ó di i r t t ó i t

Discussion
Joseph, just looking at "directed history" but hard to find definition. "Structured history" also appears. Searching has led me to another possibility though, depending on the asker's context. If we're talking about a clinical trial, it could be a structured interview (taking intencionado here to mean planned).
No sé si me explico...
"Here, Officer Fenton first inquired of Congelosi what had happened, and received no direct response. Officer Fenton then stated, "it smells like you've been drinking." In state court and in this habeas proceeding, the state has essentially argued that Officer Fenton's remark was "a spontaneous comment rather than an intentional interrogation."

Lina, as has been suggested in your other recent question: you need to provide more context and background information for your questions in order to help your colleagues help you. Sometimes a given term may be very clear to a colleague who has had experience with it; more often, we're all in the same boat as you and need as much background information as possible to try to figure out the meaning and translation of a term.
