English to Polish: Emergency Room report: Motor Vehicle Accident General field: Medical
Source text - English ABDOMEN: Soft, nontender with the exception of mild-to-moderate tenderness in the bilateral lower pelvic quadrants. There is no organomegaly here. Positive bowel sounds are auscultated throughout. There is no rigidity or guarding. Negative CVA tenderness bilaterally.
EXTREMITIES: No edema. There are no bony abnormalities or deformities.
PERIPHERAL VASCULAR: Capillary refill is less than two seconds in all extremities. The patient does have intact dorsalis pedis and radial pulses bilaterally.
PSYCHIATRIC: Alert and oriented to person, place, and time. The patient recalls all events regarding the accident today.
NEUROLOGIC: Cranial nerves II through XII are intact bilaterally. No focal deficits are appreciated. The patient has equal and strong distal and proximal muscle group strength in all four extremities. The patient has negative Romberg and negative pronator drift.
LYMPHATICS: No appreciable adenopathy.
MUSCULOSKELETAL: The patient does have pain free range of motion at the bilateral ankles, bilateral knees, bilateral hips, bilateral shoulders, bilateral elbows, and bilateral wrists. There are no bony abnormalities identified. The patient does have some mild tenderness over palpation of the bilateral iliac crests.
SKIN: Warm, dry, and intact. No lacerations. There are no abrasions other than a small abrasion on the patient's abdomen just inferior to the umbilicus. No lacerations and no sites of trauma or bleeding are identified.
DIAGNOSTIC STUDIES: The patient does have multiple x-rays done. There is an x-ray of the pelvis, which shows normal pelvis and right hip. There is also a CT scan of the cervical spine that shows no evidence of acute traumatic bony injury of the cervical spine. There is some prevertibral soft tissue swelling from C5 through C7. This is nonspecific and could be due to prominence of upper esophageal sphincter. The CT scan of the brain without contrast shows no evidence of acute intracranial injury. There is some mucus in the left sphenoid sinus. The patient also has emergent CT scan without contrast of the abdomen. The initial studies show some dependent atelectasis in both lungs. There is also some low density in the liver, which could be from artifact or overlying ribs; however, a CT scan with contrast is indicated. A CT scan with contrast is obtained and this is found to be normal without bleeding or intraabdominal or pelvic abnormalities. The patient has laboratory studies done as well. CBC is within normal limits without anemia, thrombocytopenia or leukocytosis. The patient has a urine pregnancy test, which is negative and urinalysis shows no blood and is normal.
EMERGENCY DEPARTMENT COURSE: The patient was removed from the backboard within the first half hour of her emergency department stay. The patient has no significant bony deformities or abnormalities. The patient is given a dose of Tylenol here in the emergency department for treatment of her pain. Her pain is controlled with medication and she is feeling more comfortable and removed from the backboard. The patient's CT scans of the abdomen appeared normal. She has no signs of bleeding. I believe, she has just a contusion and abrasion to her abdomen from the seatbelt and likely from the airbag as well. The patient is able to stand and walk through the emergency department without difficulty. She has no abrasions or lacerations.
ASSESSMENT AND PLAN: Multiple contusions and abdominal pain, status post motor vehicle collision. Plan is the patient does not appear to have any intraabdominal or pelvic abnormities following her CT scans. She has normal scans of the brain and her C-spine as well. The patient is in stable condition. She will be discharged with instructions to return to the emergency department if her pain increases or if she has increasing abdominal pain, nausea or vomiting. The patient is given a prescription for Vicodin and Flexeril to use it at home for her muscular pain.
Translation - Polish JAMA BRZUSZNA: Miękka, nietkliwa z wyjątkiem delikatnej do umiarkowanej tkliwości w obustronnych dolnych kwadrantach miednicy. Nie zauważono organomegalii. Podczas badania osłuchowego zarejestrowano pozytywne dźwięki jelitowe. Brak sztywności czy tkliwości. Objaw Goldfama obustronnie ujemny.
KOŃCZYNY: Brak obrzęku. Nie ma żadnych nieprawidłowości kostnych ani deformacji.
NACZYNIA OBWODOWE: Szybkość nawrotu kapilarnego wynosi mniej niż dwie sekundy we wszystkich kończynach. Pacjentka ma obustronnie nienaruszone tętno grzbietowe i promieniowe.
STAN PSYCHICZNY: Pacjentka jest świadoma, rozpoznaje osoby, jest zorientowana w czasie i przestrzeni oraz potrafi przypomnieć sobie przebieg dzisiejszego wypadku.
UKŁAD NERWOWY: Nerwy czaszkowe od II do XII obustronnie nienaruszone. Nie stwierdzono ogniskowych objawów neurologicznych. Napięcie we wszystkich mięśniach proksymalnych i dystalnych kończyn górnych i dolnych prawidłowe. Próba Romberga dała wynik ujemny, podobnie jak próba Barrego.
UKŁAD LIMFATYCZNY: Brak znaczącej adenopatii.
UKŁAD MIĘŚNIOWO-SZKIELETOWY: Pacjentka ma pełen zakres ruchu w obu kostkach, kolanach, łokciach, nadgarstkach, a także po obu stronach bioder i barkach. Przy wykonywaniu ruchów nie odczuwa bólu. Nie stwierdzono nieprawidłowości kostnych. Zaobserwowano natomiast łagodną tkliwość podczas badania palpacyjnego po obu stronach grzebienia biodrowego.
SKÓRA: Ciepła, sucha i nienaruszona. Brak ran szarpanych. Brak otarć poza niewielkim otarciem na brzuchu pacjentki tuż poniżej pępka. Nie zidentyfikowano żadnych ran szarpanych ani miejsc urazu lub krwawienia.
BADANIA DIAGNOSTYCZNE: Pacjentce wykonano liczne prześwietlenia rentgenowskie. Prześwietlenie miednicy wykazało prawidłowy obraz miednicy i prawego biodra. Wykonano również tomografię komputerową kręgosłupa szyjnego, która nie wykazała jakichkolwiek śladów ostrych urazów. Na odcinkach od C5 do C7 zauważono natomiast obrzęk tkanek miękkich przedkręgowych. Istnieje podejrzenie, że obrzęk mógł powstać na skutek uwypuklenia górnego zwieracza przełyku. Badanie TK głowy bez kontrastu nie ujawniło żadnych ostrych urazów wewnątrzczaszkowych. W lewej zatoce klinowej widoczne jest trochę śluzu. Dodatkowo, wykonano pacjentce tomografię komputerową jamy brzusznej bez kontrastu. Wstępne badania wykazały niedodmę zlokalizowaną u podstaw obu płuc. Wątroba o obniżonej densyjności, co mogło być spowodowane artefaktem ultrasonograficznym lub nakładającym się żebrami. Wykonano dodatkową tomografię komputerową z kontrastem – wynik prawidłowy, bez widocznego krwawienia ani innych nieprawidłowości w obrębie jamy brzusznej. Przeprowadzono również badania laboratoryjne. Morfologia krwi w granicach normy bez niedokrwistości, małopłytkowości i leukocytozy. Test ciążowy z moczu dał wynik negatywny, badanie ogólne moczu w normie, brak śladów krwi.
DALSZY PRZEBIEG NA ODDZIALE RATUNKOWYM: Pacjentka została zdjęta z deski ortopedycznej w ciągu pierwszych trzydziestu minut pobytu na oddziale ratunkowym. Znaczących deformacji lub nieprawidłowości kostnych brak. Pacjentce podano jedną dawkę Tylenolu przeciwbólowo, dalsza kontrola bólu poprzez podanie odpowiednich leków. Pacjentka poczuła się lepiej, zdecydowano się więc na zdjęcie jej z deski ortopedycznej. Tomografia komputerowa jamy brzusznej w normie, bez śladów krwawienia. Personel oddziału stwierdził, że doszło do stłuczenia i otarcia brzucha od pasa bezpieczeństwa i prawdopodobnie również od poduszki powietrznej. Pacjentka była w stanie wstać i poruszać się po oddziale ratunkowym bez trudu. Brak jakichkolwiek otarć lub ran.
DIAGNOZA I PLAN LECZENIA: Liczne stłuczenia i ból brzucha po przebytym zderzeniu pojazdów silnikowych. Tomografie komputerowe nie wykazały żadnych nieprawidłowości w obrębie jamy brzusznej lub miednicy. Skany mózgu i odcinka C kręgosłupa również w normie. Pacjentka w stanie stabilnym, zostanie wypisana z zaleceniem powrotu na oddział ratunkowy w przypadku wystąpienia dolegliwości bólowych lub nasilenia się bólu brzucha, wystąpienia nudności czy wymiotów. Pacjentka otrzymała receptę na Vicodin i Flexeril do przyjmowania w domu na ból mięśni.

I've got my master's degree in English Philology at the University of Zielona Góra. During that time I participated in Erasmus+ programmes in Spain and Greece where I could practice my langugage skills and learn some basics of Spanish and Greek.
Later I have become a flight attendant which I am up until now. However, I still desire to work more closelly with English language. That is why I applied for a post-graduate studies in Translation of specialized texts. I could learn there how to translate texts from such fields as Economy, Law, Medicine, Tourism etc. I found myself the most passionate about Medicine and Tourism texts, that is why I would like to work with texts from these fields.
