La sucinta nota (aggregate form) quiere decir que esta información será procesada de forma agregada, esto es, de manera que no sea posible la identificación personal del sujeto. No se me ocurre forma clara de expresarlo en castellano salvo explicitándolo. Tal vez, por ejemplo, información procesada/tratada de forma agregada.
