Spanish to English Law/Patents Law (general) service of notice
This is a form filled out by a process server in Panama.
The three choices are: "excusó notificación personal", "no quiso firmar" and "no sabe firmar".
Did the party served accept or reject or didn't require a personal service of process? It seems that "no quiso firmar" would cover a refusal to accept the notice.

.... by referring the process-server to the defendant's Procurador / Counsel, perhaps.

Excusar can mean avoid.

Otherwise, in E&W at any rate, it is not possible to 'refuse' service of process. The process-server simply confirms service or throws the writ conspicuously on the ground in front of the rebel.
Example sentence:

SPA: Lógicamente, aunque exista ese mandato de representación, si el acto de comunicación, en lugar de dirigirse al Procurador de la parte, se realiza personalmente con el interesado, será correcto y no puede *excusarse* el destinatario que no compare
