I see what you mean, Stuart. If there had been nothing to vote on, the entire column would have been left empty. There isn't the option of leaving individual boxes empty.

In this case, there definitely was a vote, so every box in the 'hlasoval' column has been filled in. The end result is that the committee (an ethics committee) has voted in favour of a particular 'substantial amendment': 'souhlasí'.

More context? I’m not sure this is “acknowledgement by one particular person”. I think it means that the whole body (e.g. committee) acknowledged a fact (say, a resignation), so all of the boxes in that specific column remain empty because there was nothing for anyone to vote on.
